Every visit is delivered by a registered nurse with experience in health checks & vital signs and is documented to support your NDIS plan and treating team.
- Full vital signs — blood pressure, pulse, temperature, respiratory rate, oxygen saturation
- Blood glucose level (BGL) checks and diabetes monitoring
- Weight, hydration, nutrition, and bowel/bladder review
- Medication review, webster pack checks, and adherence support
- Pain, mobility, falls risk, and mental wellbeing check-in
- Post-hospital discharge follow-up visits
- Escalation to your GP with a clear written summary when something isn't right
- In-home or telehealth check-ins, weekly, fortnightly, or monthly
Why this matters
A real check-in, not a tick-box visit
Our health checks are unhurried. The nurse takes your observations, reviews your medications, asks about sleep, appetite, pain, and mood, and writes it all up. You get a copy, your GP gets a copy, and your family knows where things stand. Trends are tracked visit to visit, so a gradual decline shows up as a pattern rather than a surprise.

Older people living alone, people with chronic conditions such as heart failure, COPD, or diabetes, participants in SIL/SDA settings, people recently discharged from hospital, and families who live far away and want reassurance that someone clinical is laying eyes on their loved one regularly.

Tell us what you need
A one-off check after a hospital stay, or a standing weekly or fortnightly visit — you choose the frequency and we confirm a regular nurse where possible.
The visit
The nurse completes full observations, reviews medications and general wellbeing, and answers any health questions you or your family have.
Report and follow up
You receive a written summary with your results and any recommendations. If something needs attention, we contact your GP the same day.
Funding options
NDIS funded or privately funded
NDIS participants
Health checks and monitoring are typically funded as community nursing under Capacity Building — Improved Daily Living, and may also be supported through Core Supports where they form part of daily assistance.
Privately funded
Health checks are very commonly privately funded. Pay per visit or set up an ongoing weekly, fortnightly, or monthly plan — no NDIS plan, referral, or diagnosis needed.

Can family receive the results?
Yes, with your consent. We can send the written summary to family members, your support coordinator, or your GP after each visit.
Can this be done by telehealth?
Partly. Medication review and wellbeing check-ins work well by telehealth, but physical observations require an in-home visit.
