National Continence SolutionNurse-led NDIS assessments

How it works

From first enquiry to

We keep the process simple and transparent, so participants and their teams always know what happens next.

01

Register or enquire

Complete the online registration form or call us. Participants, families, support coordinators, plan managers, and GPs can all get in touch on a participant's behalf with consent.

02

Intake and consent

Our team confirms plan details, funding availability, and consent, and gathers any relevant clinical history before the visit.

03

Book your visit

We match you with a suitable registered nurse and schedule a time that works — in your home, in the community, or via telehealth.

04

The assessment

The nurse conducts a thorough, respectful assessment covering clinical history, function, environment, products, goals, and risk. Most visits take 60–90 minutes.

05

Report and follow-up

You receive an NDIS-formatted report with costed recommendations, usually within 5–10 business days. We stay available for follow-up questions and plan reviews.

How to

Nothing is compulsory — but these few things help the nurse make the most of your visit.

  • A copy of your current NDIS plan, if you have one
  • A list of current medications
  • Details of any continence products you use now
  • A support person present, if that helps you feel comfortable

Ready when .

Registration takes a few minutes, and our intake team handles the rest.
