What is a
A continence assessment is a professional clinical review conducted by a registered nurse. It helps to understand how a participant's bladder or bowel function is affecting their daily life — and what supports, strategies, or products may help.
What's covered in an assessment
Clinical History
We take time to understand your health background, diagnoses, medications, and how continence challenges present.
Functional Assessment
We look at how you manage daily routines — including mobility, toileting independence, and existing supports.
Environment Review
Your home or living environment is considered, including access to bathroom facilities and safety factors.
Equipment & Products
We provide evidence-informed recommendations for continence products, aids, and equipment to support comfort.
Goal Setting
Assessment findings are connected to your personal goals — improving independence, managing skin integrity, or community participation.
Risk Considerations
Where relevant, we identify clinical risks (such as skin integrity concerns) and include appropriate recommendations.

What happens after?
Following the assessment, the participant receives a professional, NDIS-formatted written report. This comprehensive document is designed to be submitted directly to the NDIA.
