What we collect
We collect the personal and health information needed to deliver nurse-led continence assessments and community nursing care — including your contact details, NDIS plan information, clinical history, and the notes made during your care.
How we use it
Your information is used to plan and deliver your care, prepare assessment reports, communicate with the people you nominate, and meet our clinical record-keeping obligations. We never sell your data.
Who we share it with
We only share information with people you have consented to — such as your support coordinator, plan manager, GP or treating team — or where we are required to by law.
Access and corrections
You can request access to the information we hold about you, or ask us to correct it, at any time by contacting our team.
