A coordinator's checklist for continence assessments
Everything support coordinators need to gather before referring a participant — so the assessment is funded, scheduled and reported without rework.
Before you refer
Confirm the basics in five minutes:
- Plan dates and the participant's NDIS number.
- Plan management type (self, plan or agency managed).
- Available funding under Capacity Building – Improved Daily Living and/or Core – Nursing.
- Consent to share clinical information with our team and any third parties (GP, OT, equipment supplier).
What we will ask for
To prepare the report, we usually need:
- A copy of the current NDIS plan (or the relevant pages).
- Recent allied-health reports (OT, physio, dietitian) where they exist.
- Any equipment lists or supplier contacts already in use.
- A point of contact for the participant — themselves, a family member, or your team.
Booking the assessment
We work in-home, in the community and via telehealth. To book efficiently, share:
- The participant's preferred days and times.
- Address and any access notes (parking, intercom, key safe, support animals).
- Whether you, a family member or another professional will attend.
After the assessment
We will deliver a written report within 5 to 7 business days. The report is structured to support funding requests and includes:
- A clinical summary.
- Recommended products, routines and equipment.
- Goals aligned to the participant's plan.
- Suggested supports for the next plan period.
If anything in the report needs clarification, we are happy to dial in to a planning meeting at no extra charge.
